Home Health Services are comprehensive, and tailored to the specific needs of each patient within the comfort and security of their own home.

Some of the services provided by Promise Care NJ's bilingual staff include:

Skilled nursing services:

  • Maternity (ante and post partum) High-Tech, Wounds, Ostomies, Pediatric, Psychiatric, Medical, HIV +/ Aids, Geriatric, to name a few
  • Clinical Social Workers
  • Rehabilitation Therapists (Physical, Occupational, Speech)
  • Nutritional care
  • Home Health Aides

Home Health services are provided to recovering, disabled, or chronically ill persons who need medical treatment and/or assistance with the activities of daily living. The services provide a continuity of care for patients referred from hospitals, physician offices, Emergency Rooms, Ambulatory Care Centers, clinics, Skilled Nursing facilities, and community agencies.

Promise Care NJ has liasons that regulary visit many of "Hudson County's" hospitals, Skilled Nursing facilities, Rehabilitation centers, etc. Our nurse liasons all have a clinical background and can visit you in the hospital or rehab facility to explain Home Care to you.

All referrals are responded to by a Registered Nurse in our Intake Department. The nurse will also communicate with the attending physician, case managers or insurance representative.
