Home Health Services

Home Health Services are comprehensive, and tailored to the specific needs of each patient within the comfort and security of their own home.







Some of the services provided by our staff include:

  • Specialty nursing services: Maternity (ante and post partum) High-Tech, Wounds, Ostomies, Pediatric, Psychiatric, Medical, HIV+/AIDS, Geriatric
  • Clinical Social Workers
  • Rehabilitation Therapists (Physical, Occupational, Speech)
  • Nutritional Care
  • Home Health Aides

Home Health Services are provided to recovering, disabled, or chronically ill persons who need medical treatment and/or assistance with the activities of daily living. The services provide a continuity of care for patients referred from hospitals, physician offices, Emergency Rooms, Ambulatory Care Centers, clinics, and community agencies.

All referrals are responded to by a Registered Nurse or our Intake Department. The nurse will also communicate with the attending physician and case managers of insurance representatives.
