Clinical Social Work Service
 Promise
Care NJ LLC, Clinical Social Work Services are available to home
health patients in order to address the emotional, social, and
environmental issues which may interfere with the optimal response
to medical care and home care services. Goals are developed with
the patient and/or caregiver, and services are provided in accordance
with the physician plan of care.

- Counseling, Short-term Psychotherapy, Family Therapy
- Crisis Intervention
- Long-term Care Planning
- Community Resource Information and Referral
- Mediation for Family
